Good Faith Estimate Notice

You have the right to receive a Good Faith Estimate explaining how much your medical care may cost.

Under federal law, health care providers must give patients who do not have insurance or who are not using insurance an estimate of the expected charges for qualifying scheduled health care services.

Who can request a Good Faith Estimate?

You may request a Good Faith Estimate if you are uninsured or if you choose not to use insurance for qualifying scheduled health care services.

You may ask for an estimate before scheduling a service or at any time before receiving care.

What the estimate may include

A Good Faith Estimate describes expected charges for qualifying scheduled services provided by CuraNow Health. It is based on the information available when the estimate is prepared.

The estimate is not a contract and may not include unexpected services, testing, referrals, or other care that becomes medically necessary during your evaluation or treatment.

Third-party services

Charges from pharmacies, laboratories, imaging centers, hospitals, specialists, or other third-party organizations are separate from CuraNow Health’s professional fees unless otherwise stated.

CuraNow Health cannot provide a Good Faith Estimate for services billed directly by another organization.

How to request an estimate

To request a Good Faith Estimate for a qualifying scheduled service, contact CuraNow Health before your appointment. Please do not include diagnoses, symptoms, medications, or other sensitive medical information in a general website form or regular email.

Medical information should be communicated only through CuraNow Health’s designated secure clinical platform.

If your bill is substantially higher than your estimate

If you receive a bill that is at least $400 more than your Good Faith Estimate, you may have the right to dispute the bill through the federal patient-provider dispute resolution process.
